Criminal Record Check 

(Act 34 Background Clearance)
Only original document with State Seal Watermark accepted

  • The Criminal Background Check can be obtained online using a debit or credit card for application fee at https://epatch.pa.gov/home
  • Is good for one year from the date the clearance has been granted.
  • Students must check “Other” under Purpose of Clearance
  • Must say “no record”
  • May take two to four weeks to be processed.
  • Applicants must make sure that they print the “official” Certification Form with the State Seal Watermark in the background.

Pennsylvania Child Abuse History Clearance 

(Act 151)
Only Originals Accepted

  • Is good for one year from the date the clearance has been granted
  • Students must check “School Employment Governed by Public School Code” under Application Purpose
  • Must say “no record”
  • Child Abuse History can obtained online at https://www.compass.state.pa.us/cwis/public/home
  • Login or Create an Account: Follow directions. Note that following information will be needed before you start to help you complete your application:
      • Addresses where you have previously lived
      • Names of all individuals with whom you have lived to include parents, guardians, siblings, spouses, etc.
      • Any previous names you have used or have been known by
      • Credit Card (only) information for application fee.
      • Follow directions and you will receive results via e-mail; you will be able to print the results. The processing of this clearance may take up to 3-4 weeks so plan accordingly.

FBI Clearance 

(Act 114)
Only Originals Accepted  

  • All student teachers, students participating in a classroom, and anyone employed in private or public schools must complete a current FBI Federal Criminal History Record.
  • This clearance is good for the entire time you are enrolled at LHU; unless you lose the original or if you have a break in your enrollment.
  • Must say “no record”
  • This clearance must come from the Department of Education. Our office will not accept clearances from the Department of Human Services!
  • You must be registered with IdentoGO at: https://uenroll.identogo.com/ using code 1KG6RT prior to having your fingerprinting done.   You can also register by phone by dialing 844-321-2101. 
  • Enter the link indicated above into your web browser.  This website will walk you through the process and show you the fingerprinting locations sites.  An instruction sheet is available here.
  • If using Lock Haven Public Safety, please refer to the instruction sheet link.
  • It may take 4 weeks to receive your clearance results. You will not be permitted to enter schools without it; so plan ahead.

Tuberculin (TB) Test

Only Medical Reports Accepted

  • Is good for two years from the date of the test. If a student has a valid TB test on record at the LHU Glennon Infirmary, the student may go there to request a TB Test Verification Form. (New students must have the TB test as part of their Medical Record for entrance to the University, so it will be part of their physical examination form and we recommend that you keep a copy of that TB Test Verification information to submit to our office.)
  • If a student needs a TB test, the Infirmary will do the test free of charge on weekdays, except Thursdays.
  • The TB Test verification from the infirmary or from a private physician must include the student’s name, date of test, and the test results (which should be negative), date results were read and the physician’s name on the report.

Act 24 Arrest/Conviction Report and Certification Form

  • Self-reporting clearance that is required by some school districts.  Please print, complete, sign, and keep with other clearances.

Mandated Reporter Training

(Act 31)
Certificate of Completion required as part of the Student Teaching Application

  • All students are required to complete the on-line training as part of their Student Teaching Application requirement.
  • This training is good for the entire time you are enrolled at Lock Haven University, unless you have a break in your enrollment.
  • The 3-hour free training can be found at Mandated Reporter Training. You must first register and then take the course.
  • A copy of the completion Certificate must be submitted to the Office of Student Teaching, Clearance Coordinator, (RLC 609).

Note:  Non-U.S. citizens must apply for clearances and a TB test as well.
